Introduction

To access the 2024-2025 seasonal year, member clubs must submit the Annual Affiliation Form as a part of the annual registration process.  This form is an official method of communicating key information from the Member Club to New Jersey Youth Soccer. Upon completion, NJYS will update the status of your club for access to the new seasonal year within 2-3 business days.

Please have the following information available to you before completing this form:

1. Contact Information for Club Board Members and/or Administrators 
2. IRS Corporation and Tax-Exempt Status (If Applicable)
3. Online Registration Provider Information (If Applicable)

Note: Insurance certificates for the new seasonal year are distributed on an ongoing basis beginning on or about August 1, 2024. Insurance certificates for the 2023-2024 seasonal year are valid through August 31, 2024.
